起草美国护照与中国护照为同一人证明

注意起草声明书时，如果有两页，一定要将每页添加页码，并在声明书中说明（见附件一，第1页）（见附件二，第2页），否则第2页有可能被认为是在公证后又自行加进来的，因而导致文件被拒收。

特此声明，美国护照与中国护照均为我本人所有，系同一人，我保证以上内容属实，如有不实之处,本人愿承担由此所引起的一切法律责任。Being duly sworn in, hereby solemnly declare that the ownership of U.S passport attached and Chinese passport attached are the same person. I declare, that the foregoing is true and correct. I am the person named above, and I assume full responsibility for the declaration.
