Document Checklist
Read Carefully!
Required:
- Need passport and its photo page copy.
- Visa Form. All pages MUST BE TYPED, EVERY ITEM FILLED OUT or N/A. Please make sure to sign all the right places in wet signature.
- One formal 2×2 passport photo taken within 6 months.
- For Part 1.9: use Driver’s License # AND mail in a copy (kid may leave this blank).
- For Part 1.15 Occupation: AVOID “Businessman”.
- For Part 1.17 Company name, show business type. Must sign visa form section 4.
- For kids below 18 years a parent must sign section 4. Also do section 5.
If Non-US Passports:
- A doc for legal and long term stay in US, like US Citizen Card, TN/TD/H visa/student I-20 form, etc. A clear copy (Note: San Francisco needs original).
Keep trip purpose simple, avoid mix more than one.
- For L-Tourism: Must have A) tourism agent full agenda OR, B) Round trip air ticket & hotel reservation.
- For Q1/Q2 Family Visiting: need C) Invitation Letter from inviting individual (with China ID card front and back).
- F-Business visa: Visa Authorization Letter by Chinese government or US employer letter with letterhead, PLUS China Invitation.
- X-Student visa: Admission letter plus visa authorization letter by government.
- Z-Working visa: need “Foreign Expert Work Permit” plus visa authorization letter from government.
Please print 2 copies of your order. Keep one for record and mail the other one to us.
Make a copy of your passport before mailing it out. Keep it safe.
